Clinical Study of the Benign Biliary Tract Diseases

Abstract
|
|
|
An analytic review of the clinical records of 75 cases of benign biliary tract diseases who were treated surgically at Dept. of General Surgery of Pu Cheon Se-Jong General Hospital for 4 years from Aug. 1982 to Jude 1986 was to get following results.
1) The causative diseases were inflammatory disease c stones which were most common in 62 cases (82.6%) and follows inflammatory disease s stones in 5 cases (6.6%), clonorchiasis in 4 cases (5.3%), ascaris in CBD in 3 cases (4%), and congenital anomaly of GB in 1 case (1.3%) respectively.
2) The age group of the highest incidence was in 5th decade in male and 4th decade in female. The male to female sex ratio was 1:1.4
3) The prominant symptoms were epigastric pain in 38 cases (50.6%), pain on R.U.Q in 31 cases (41.3%), and radiating pain in 25 cases (33.3%) respectively. And the prominant physical signs were tenderness in 42 cases (56%), positive Murphy¢¥s sign in 25 cases (33.3%), palpable GB in 6 cases (8%) and hepatomegaly in 4 cases (5.3%).
4) The positive stone shadow was identified in 9 cases (15.7%) on the plain abdominal films, and 62 cases (82.6%) on the ultrasonography.
5) Stone was identified in 62 cases (82.6%), the most frequent site was gallbladder in 34 cases (54.8%), and followed by intrahepatic duct in 12 cases (17.1%), common bile duct in 10 cases (16.1%) and GB & CBD in 6 cases (18%).
6) In the operative procedures, cholecystectomy was performed most frequently in 36 cases (50%), cholecystectomy c T-tube drainage was in 29 cases (40.2%) the next, and cholecystectomy c choledochojejunostomy was in 9 cases (12.5%).
7) Postoperative complications were noted in 25 cases (33.3%). Residual stones was the most common in 10 cases (13.3%), and bile leakage in 3 cases (4%) the next.
8) The overall mostality was 2 cases (2.7%) who were performed in emergency operation, and the cases of death was sepsis in 1 case and the hepatorenal syndrome in 1 case.
